What To Expect: Prior
to your inital visit you should have contacted Debbie at 262-598-9030
to ensure insurance coverage. Debbie will be more than happy to assist
you in determining your benefits.
During your inital evaluation,
Dr. Jacobson will be asking you a lengthy series of questions to
determine your appopriate diagnosis and treatment plan. Psychiatry is a
medical specialty. Although the first session may involve some degree
of patient education and therapy, its primary purpose is to assure that
you are diagnosed as accurately as possible in the time alotted. If
your inital evaluation takes more than the alotted time, you may be
asked to return for a second evaluation visit before treatment is
initiated.
Preprocedure: Prior
to your visit you will be mailed a packet of information to fill out.
It is essential that this information be filled out and brought to your
appointment. This packet will include forms for registration, office
policy forms, and a lengthy medical and psychiatric history form. As it
is a lengthy form, you will be unable to fill it out it out in the
office prior to your visit, so please plan ahead.
Postprocedure: Follow-up
appointments will be scheduled as determined by Dr. Jacobson. Please
make these immediately after your session with him if possible. This
will assure that you will have an appointment in a timely fashion and
before your medications run out. Certain medications can be called in
in an emergency if necessary. Others, however, cannot be. Please plan
ahead.
If you have an emergency after hours, please be sure to
go to the nearest emergency room. If it is something that can be
handled by phone, you may call Dr. Jacobson's office number for further
information about how to reach him after hours and about office policy
concerning this.
What To Expect: These
appointments are for follow-up to assess how you are doing and to
readjust you medication if necessary. Minimal psychotherapy will be
provided. If the time od the session extends beyond the time alloted by
your insurance company, you may personally be responsible for any extra
charges incurred.
Preprocedure: Please
come prepared with a full list of all medications that you are
currently taking, as these may have changed since your last visit. This
will assist Dr. Jacobson in prescribing appropriate medications for
you. This is impportant because psychiatric medications often interact
with other prescibed medications, over-the-counter medications, and
sometimes herbal and nutritional supplements.
Postprocedure: You
will be given a prescription to cover you until your next appointment.
It is very time- consuming for our office to have to call in
prescriptions. Please extend to us the courtesy of keeping your
appointments on time so as to minimize this. Thank you.
Staff Information
Debbie
is our office manager. She will answer any questions you may have
regarding insurance, calling in presciptions, billing and payments, and
patient assistance programs and forms. All phone calls during working
hours should go to Debbie first and she will triage them to Dr.
Jacobson accordingly. If the issue is an emergency, please notify
Debbie as to the nature of the emergency so that she can pass the
information on to Dr. Jacobson. Dr. Jacobson will decide whether an
issue is truly as emergency and will get back to you accordingly.
Attempts are made to respond to phone requests the same day that they
come in. However, sheer volume of phone calls sometimes prevents this.
Dr. Jacobson encourages patients to set up appointments within the time
frame that he has recommended so that medications can be filled during
office visits. Under no circumstances will prescriptions for highly
controlled substances be called in after hours or on non-working days.
WHen
in the office, please make every effort to allow Debbie to complete her
job. She has many tasks to do on any given day and your cooperation in
this matter is appreciated.
Please note: Any patient who
verbally abuses Debbie or any other staff member in this office will be
immediately terminated from this practice. Any threats against staff
will be turned over to the police if they represent a true danger to
our staff.
